What we collect
Only the details needed for a basic appointment request.
The form asks for your name, phone number, optional email address, whether you are a new or existing patient, the general type of visit, a preferred time and consent to be contacted. Please do not enter medical, dental, insurance or other sensitive information in this form.
The server also processes limited technical information, such as an IP address and request details, for security, troubleshooting and short-term rate limiting. Standard server logs may record page requests.

You may request an appointment by calling 604-558-8381. You can also ask the clinic about access to or correction of personal information in its custody, subject to identity verification and applicable law.
